Application Period: 12th to 23rd March 2012

1) Application form

2) Learning Agreement

3) Self-Graduation Checking List

4) Recommendation Letter (from Academic Staff)

5) Study Plan

6) Course Description (for courses not listed in the Course Selection Template)

7) One photo

8) Copy of passport
(Students from mainland China should also submit a copy of Exit-entry Permit for Travelling to and from HK and Macau)
